Using a holistic and client/family centered approach, the incumbent facilitates appropriate assessments, referrals and placements of clients with long term mental illness as well as implementing mental health home care services for discharged patients. The Senior Assessor Coordinator facilitates assessments, referrals, and placements for mental health clients to ensure appropriate community residential options. They also implement mental health home care services for discharged patients using a client-centered approach.

Candidates must hold a Bachelor of Science in Nursing, Bachelor of Social Work, or Bachelor of Psychiatric Nursing, or equivalent diplomas. A valid Class 5 driver's license and registration with the appropriate licensing body are also required.
