POSITION SUMMARY: THP is seeking an experienced Quality Improvement Advisor to join the Quality Improvement (QI) team within the Quality and Patient Safety Department, with a dedicated focus on supporting the development, implementation and ongoing operations of the Trillium Operations Centre, interface with Trillium Connect, and other Reducing Hospitalization initiatives as assigned. Trillium Operations Centre and Trillium Connect are two enterprise-wide initiatives that will transform how, when, and where we deliver care with the goal of reducing hospitalization through digitally-enabled seamless and connected care. This role represents a unique opportunity to help map and operationalize the processes needed to launch a real-time AI enabled operational control hub that provides complete system-wide situational awareness to optimize flow and capacity, as well as the processes to allow this hub to interface with THP’s digital front-door that will deliver seamless, continuous, personalized care. Working closely with clinical and operational leaders, and other enabling partners, the successful candidate will help develop and optimize processes, establish measurement frameworks, and translate operational signals and AI-enabled insights into testable, measurable, and sustainable improvements. These efforts will improve patient flow, reduce avoidable hospitalizations, and enhance both safety and clinical quality. Reporting to the Manager, Quality and Patient Safety, the Quality Improvement Advisor will lead work that integrates quality improvement science, learning system principles, and evidenced-based practice in direct support of THP’s Plan to 2030 and Quality Improvement Plans. We are looking for a high-level, certified change agent who has experience leading both complex, cross-functional improvement projects and reduction of waste and variability to improve operational performance. ACCOUNTABILITIES: * Function as a lead for enterprise-wide or Program/Department-specific level quality improvement initiatives that directly support Trillium Operations Centre, working with interdisciplinary, multi-site teams to advance THP’s Plan to 2030 and Quality Improvement Plans. * Identify gaps and strategize improvements by using Lean methodologies to enhance operational efficiency, patient flow, and quality of care across programs and THP’s Operations Centre related processes. * Integrate quality improvement and learning system principles into workflows and escalation practices to ensure THP Operations Centre functions proactively. * Facilitate large process mapping sessions and develop accountable action plans to guide key partners through change. * Serve as a connector among initiatives aimed at reducing hospitalizations and related system-level workstreams, ensuring alignment, prioritization, and consistent improvement approach * Manage multiple project goals, timelines, milestones, and resources in collaboration with hospital programs, and enabling services to drive effective and sustainable improvements. * Lead and support the coordination and facilitation of improvement projects using project management fundamentals. * Educate, coach, and lead knowledge translation to support point of care and enabling services staff during the implementation of quality improvement initiatives, including those influences by THP Operations Centre insights. * Identify, develop and track key quality, patient safety, and operational performance metrics in partnership with key enabling partners to drive system-level performance, as well as providing regular work plan updates to varying levels of leadership. * Other duties as required to support the advancement of THP’s Command Centre. QUALIFICATIONS: * An undergraduate degree in industrial engineering, business, life sciences, or a health-related discipline is preferred, or an equivalent combination of education and relevant experience. * Lean Six Sigma Black Belt certification is strongly preferred. Candidates with significant experience applying Lean methodologies and who are working toward certification may also be considered. * A Master’s degree in quality and/or patient safety, business administration, public health, healthcare administration, or a related field is considered an asset. * Several years of experience working with complex, dynamic teams in healthcare or a related sector is preferred. * Demonstrated experience supporting process and/or quality improvement initiatives, particularly those involving interdisciplinary and cross-program collaboration, is an asset. * Proven ability to build and maintain effective working relationships with diverse teams and key partners. * Familiarity with project management principles and experience coordinating projects or improvement initiatives. * Ability to organize and prioritize work while managing multiple tasks in a fast-paced environment. * Strong interpersonal and communication skills (presentation, written, and verbal), with the ability to share information and insights with a range of audiences including leaders, administrators, and clinicians. * Well-developed strategic thinking and problem-solving skills. * Strong analytical abilities, including experience working with qualitative and quantitative data to support decision-making and improvement efforts.
