Job Description
The Program Coordinator (PC) provides support and coordination to the daily operational activities of a
program. The PC assists in the support of individuals in care, with the activities of daily living and in
keeping with their individual person-centered plans. The PC will also provide decision support to the
team and liaison with clinical professionals in the absence of the Program Manager.
The Program Coordinator will respect and uphold the Values, Vision and Mission of Karis Disability
Services, demonstrating the highest standards of courtesy and respect in their dealings with other
employees, people using services and both internal and external stakeholders.
Direct Care Supports to Individuals
- Maintain a flexible schedule (including working weekends) to maximize presence in the program
when the Program Manager is not present.
- Provide direct supports to promote well-being and enhanced quality of life consistent with Person
Centered Support.
- Provide direct support with personal hygiene (including toileting, dental care, and lifting), dressing,
meals, medication, health treatments, social and leisure activities, and transportation as needed.
- Implement person-centered planning to facilitate fulfillment of individual goals and dreams.
- Empower individual choice and promote access to natural/community supports.
- Provide support in a manner that recognizes and supports the spiritual needs of each person
according to their choices.
- Provide crisis/emergency supports.
- Perform household maintenance duties.
- Complete menu planning, meal preparation, housekeeping duties, laundry, home décor, small
repairs, vehicle maintenance, shopping, yard work, and medications supply management.
- Develop a home-like environment to empower individual choice and participation.
Indirect Care Supports to Individuals
- Support the team and Program Manager in the implementation and compliance to policies and
procedures of Karis Disability Services in the program.
- Meet the requirements for in-charge/emergency on call duties as defined by program and district.
- Support the team in effective documentation (supports notes, health records, quarterly reports,
exceptional occurrence).
- Meet with the Program Manager to plan, address program concerns, problem solve and evaluation.
- Coordinate to ensure effective communication with families, professional and community members.
- Develop and implement in-house orientation procedures for new Support Workers, seasonal
employees, and volunteers.
- Refer and consult with community professionals such as Physicians, Specialists, Psychologists, Social
Workers, Behaviour Therapists, Teachers, Employment personnel, ODSP, Karis Disability Services
Clinical Director, etc., as directed/requested.
- Coordinate the team’s activities to facilitate treatment plans and professional recommendations are
communicated to and implemented by the team, in a consistent manner.
- Coordinate access to community activities/leisure programs and other programs/volunteers.
- Summarize Occurrence reports monthly for follow-up with the Program Manager on unresolved
issues.
Self Managing Work Team Support
- Assist the facilitation of the functioning of tasks, in keeping with Karis Disability Services policies and
procedures.
- Assist the activities of the Program Manager in training, equipping and coaching Coordinators and
other team members having specific areas of responsibility (e.g., Prime Counsellors).
- Assist the activities of the Program Manager in the orientation and training of new program staff,
seasonal employees, and volunteer placements, within the policy parameters and values of Karis
Disability Services.
- Chair team meetings as assigned, including any preparations and follow up.
Experience: Minimum three years of related experience
Skills Required:
- DSW or CYS Diploma preferred
- Current/Valid G Driver's License and eligible to drive Karis Vehicles
- Computer Skills: Windows, Word, Excel, Email, etc.

- Supports to people who have a dual diagnosis
Ability Required:
- Current NVCI
- Current First Aid/CPR
- Able to lift _35_ lbs
- Able to perform restraints
- Able to perform CPR
- Able to provide personal care which may include lifting, bending and/or twisting
- Must be able to drive. Must have a Full G Driver's Licence
Shifts are 7h -12h, alternating weekends, days and evenings, 35 hours per week.
Week 1: Mon 10-17, Tues 10-17, Wed 8-15, Thurs 8-15, Sat 8-15,
Week 2: Sun 8-15, Tues 8-15, Wed 8-15, Thurs 10-17, Fri 16-23
The successful candidate will be selected based on Article 13.03 of the UFCW Toronto-York Bargaining Unit Collective Agreement.
