Position Summary: 

  • Works in collaboration with multi-disciplinary seniors mental health team
  • Provides outreach and virtual visits to seniors with major mental illness in the Community, Long Term Care homes, and Retirement Homes
  • Engages and consults with community referrers, service providers, clients and families and to create collaborative care plans
  • Supports clients and families and community partners to achieve goals as set out by care plan
  • Opportunities to participate in Seniors Mental Health planning, by working in partnership with system resources such as within the Mental Health Program and other THP programs, Primary Care Providers, community groups/agencies including Home and Community Care, Public Health to identify and jointly work on system level change
  • Act as a primary health care consultant to health systems and departments
  • Contributes to program planning, sits on various committees and community groups
  • Maintain current health profiles for resources which serve Trillium communities

  • Demonstrates commitment to best practice guidelines and quality improvement
  • Takes initiative to tackle important issues, problems, and tasks pro-actively
  • Commitment to vision and goals of THP mental health strategic plan

 

Key Qualifications:  

  • Registered Nurse with current registration with the College of Nurses of Ontario and in good standing 
  • Graduate of a recognized College or School of Nursing, with Baccalaureate degree preferred 
  • Combined with current, demonstrated seniors mental health clinical experience, ideally 5 years' experience but willing to train the right individual 
  • Ability to recognize and intervene in clinical and psychosocial situations where management of Behavioral and Psychological Symptoms of Dementia (BPSD) is required
  • Sound working knowledge of the Mental Health Act, Public Hospital Act, Long Term Care Act, Substitute Decisions Act, and the Regulated Health Professions Act required
  • CNA certification in Psychiatric Mental Health Nursing and/or Gerontological Nursing an asset or willingness to acquire within 2 years of employment
  • Strong knowledge and principles of Crisis Intervention
  • Experience in community and outreach programs
  • Knowledge of relevant medications and their side effects
  • Must provide own transportation and a valid driver's license with access to a dependable vehicle as the job will require that you drive within Mississauga Halton LHIN
  • Excellent interpersonal, communication, and organizational skills combined with the ability to work independently as well as part of an inter-professional team
  • Ability to prioritize competing demands, positive work and attendance record required
  • Knowledgeable with data collection tools
  • Current CPR and BSL certification required for Nursing role
  • Hospital based experience is considered an asset
