Job Description: Reporting to the Primary Health Care Manager, the Primary Health Care Nurse is an integral member of the primary health care team, providing and coordinating initial, continuing and comprehensive nursing services to patients in the PHC sites. The Primary Health Care Nurse promotes quality health care outcomes working within established standards for case management practice. The Primary Health Care Nurse initiates and/or participates in community development initiatives and participates in the evaluation of Primary Health Care initiatives within the site.

Human Resources Exemption: No

Experience

  • Experience in case management is an asset
  • 2 years nursing experience in a community based health care setting preferred
  • Experience in teaching adults, facilitation skills, and leading group activities

Education

  • Bachelor Degree or Diploma in Nursing or Psychiatric Nursing

Licenses

  • Basic Life Support Certificate (BLS)

  • Knowledge in chronic disease prevention and management
  • Strong computer skills in Microsoft Office, scheduling & training platforms
  • Familiar with the principles of population health promotion
