Applicants are encouraged to self-declare in their application.  Position Summary The Primary Health Nurse II provides health education and care within the scope of nursing practice. As a member of a client-centered interdisciplinary team, the Primary Health Nurse II provides care to individuals promoting wellness, prevention of disease and/or disability and the management of chronic diseases/conditions.
