The position is compensated according to an established wage grid, with progression through the steps occurring in accordance with applicable policies until reaching the maximum of the salary range.

POSITION OBJECTIVE: Niagara Health is seeking compassionate, community-minded individuals to join our Indigenous Health Services and Reconciliation team in supporting Indigenous individuals and families who have been displaced from their home communities. This role is about much more than responding to an emergency, it’s about providing culturally safe, compassionate care during a time when it is needed most.

This temporary opportunity offers the chance to make a meaningful and immediate impact by helping Indigenous patients and families navigate their healthcare journey, access services, and receive culturally responsive support while away from their communities.

Reporting to the Manager, Indigenous Health Services and Reconciliation, and working collaboratively with the interdisciplinary team, the successful candidate will provide advocacy, coordination, and support for Indigenous patients and their families throughout their care experience. This includes facilitating access to Niagara health services, assisting with care transitions, and collaborating with community partners to promote seamless, patient-centered care.

The successful candidate must be willing to work across various NH sites and participate in an on-call rotation. Flexibility to work days, evening, weekends, and holidays is required to ensure timely support for the communities.

RESPONSIBILITIES: o Maintain a high level of confidentiality and safety in support of a respectful, inclusive and compassionate care journey for Indigenous patients, their families and caregivers, and members of their care team o Facilitate communication with the patient/family and all members of the care team to promote a culturally safe care environment and the delivery of high-quality patient care o Facilitate the intake and follow-up of Indigenous patient experience concerns/ complaints/ questions and work with the care teams to achieve resolution for patients and families o Facilitate access to traditional healing practices including, cultural and spiritual practices for patients and families at their request o Work collaboratively and build relationships with all Indigenous healthcare providers and organizations in the region to ensure continuity of care and improve transitions from hospital to home or community care environments o Work collaboratively with other Indigenous Navigators across the care continuum to facilitate access and the coordination of care o Develop and maintain an inventory of Indigenous health and social services in Niagara to be utilized by all care team members involved in discharge planning o Align improvement work with Niagara Health's Strategic Plan by identifying, monitoring, and implementing an action plan in support of the Indigenous patient journey o Participate in education and training specific to current, relevant federal and provincial health and safety legislation, standards and guidelines

QUALIFICATIONS: o Four (4) year Bachelor's Degree in Health Sciences, Social Sciences, or in a related field, from an accredited university would be considered an asset o Three years of navigation, case management, or other similar experience in healthcare or the social services sectors o Lived experience (must self-identify as Indigenous) o Strong knowledge of the history of Indigenous Peoples and the impact of intergenerational trauma o Familiarity with system navigation models and principles o Knowledge of local Indigenous organizations and community resources o Knowledge and experience with traditional ceremonies and healing practices o Ability to perform some culturally relevant ceremonies would be an asset o Excellent communication skills (both written and oral) o Superior interpersonal, collaboration, facilitation, and negotiation skills o Strong problem-solving abilities o Demonstrated ability to work effectively in a multi-disciplinary team environment o Possession of a valid driver's license and access to a reliable vehicle o Consistent and regular attendance required
